Health Insurance Glossary for Benefit Verification Teams
This glossary is designed for behavioral health facilities, treatment centers, and private practices that verify insurance benefits for prospective patients. It explains key insurance terms you'll encounter when confirming eligibility, coverage, and financial responsibility for various healthcare services.
Affordable Care Act (ACA): Federal law requiring most insurance plans to cover mental health and substance use disorder services at parity with medical care (no greater limits or cost sharing).
Allowed Amount: The maximum payment an insurance plan will recognize for a covered service. For in-network providers, this is the contracted rate; for out-of-network claims, it's often the "usual and customary" rate.
Authorization (Preauthorization / Prior Authorization): Insurance approval required before certain mental health services are rendered (e.g., inpatient, PHP, IOP, or extended therapy sessions). Always verify whether preauthorization is needed during the benefit check.
Behavioral Health: A broad category that includes both mental health and substance use disorder treatment services.
Benefit Period: The timeframe during which benefits apply, usually a calendar year or policy year. Deductibles and out-of-pocket maximums reset at the start of each new period.
Claim: A billing request submitted to an insurance carrier after a service has been provided. It details what was done, the cost, and the provider's information.
Client Responsibility: The portion of the bill the patient is responsible for after insurance processing (includes deductibles, coinsurance, and copays).
Coinsurance: The percentage of allowed charges the patient pays after the deductible is met (e.g., 20% patient / 80% insurance).
Copayment (Copay): A fixed dollar amount the patient pays for a covered service, typically due at the time of service.
Coverage Limitations: Specific restrictions on mental health services, such as visit limits, non-covered services, or medical necessity requirements.
Deductible: The total amount the patient must pay out of pocket for covered services before insurance begins paying. When verifying benefits, note both the deductible amount and how much has been met to date.
Eligibility: Verification that a patient's insurance policy is active and valid as of the service date. Always confirm start and end dates and check for terminations or COBRA coverage.
Employee Assistance Program (EAP): Employer-sponsored programs that often cover a limited number of free or low-cost sessions before regular insurance benefits apply.
Explanation of Benefits (EOB): A statement issued by the insurer after a claim is processed, showing billed charges, allowed amounts, what was paid, and what the patient owes.
Formulary: The list of medications covered by a plan. Relevant when verifying psychiatry or medication management benefits.
Facility-Based Services: Higher levels of care like inpatient, residential, PHP (Partial Hospitalization Program), or IOP (Intensive Outpatient Program). Often require preauthorization and clinical documentation.
Health Maintenance Organization (HMO): A plan type that requires members to use in-network providers and may require PCP referrals. Out-of-network mental health care is typically not covered.
Health Savings Account (HSA) / Flexible Spending Account (FSA): Pre-tax funds patients can use to pay for qualified healthcare costs, including mental health treatment.
In-Network Provider: A provider or facility contracted with the insurance carrier. In-network status affects reimbursement rates and patient responsibility.
Level of Care (LOC): The intensity of treatment needed, such as Outpatient, IOP, PHP, or Residential. Coverage and preauthorization rules often vary by LOC.
Managed Care: A system where insurers use networks and utilization review to control costs and ensure services are medically necessary.
Medical Necessity: A determination that a service is clinically appropriate and required. Insurers use this standard to approve or deny mental health treatment.
Member ID / Subscriber ID: Unique number used by the insurance company to identify a plan participant.
Network: The group of providers contracted with an insurer. Always confirm if your facility or provider is in-network when verifying benefits.
Non-Covered Services: Services excluded from coverage under the policy (e.g., certain psychological testing or couples therapy).
Out-of-Network (OON): Providers not contracted with the patient's insurance plan. Coverage may still exist but typically at lower reimbursement and higher patient cost.
Out-of-Pocket Maximum (OOP Max): The total amount a patient pays for covered services during a plan year. Once reached, the plan covers 100% of allowed costs.
Outpatient Services: Standard mental health sessions (therapy, medication management, telehealth) provided without admission to a facility.
Plan Year: The 12-month period for which benefits apply (may differ from the calendar year).
Precertification: Another term for preauthorization; required approval for specific mental health services prior to treatment.
Premium: The monthly cost paid by the member or employer to maintain coverage.
Primary Insurance / Secondary Insurance: When a patient has multiple policies, the primary plan pays first, and the secondary plan may cover the remaining balance.
Provider Type: The classification of the rendering provider (e.g., LPC, LCSW, PsyD, MD). Some plans only reimburse certain types of providers.
Referral: A written or electronic approval from a primary care physician allowing a patient to see a mental health specialist, often required by HMO plans.
Substance Use Disorder (SUD): A category of behavioral health treatment for drug and alcohol-related conditions. Verify whether coverage includes detox, residential, and outpatient programs.
Subscriber: The primary person covered under the plan (e.g., employee or policyholder). Dependents are listed under the subscriber's policy.
Telehealth / Teletherapy: Services delivered via video or phone. Most commercial plans now cover these at parity with in-person visits, but verify place-of-service codes and modifiers when checking eligibility.
Treatment Plan: A provider-created document outlining goals and interventions. Some insurers require periodic treatment plan updates for ongoing authorization.
Utilization Review (UR): The insurer's process for reviewing ongoing treatment to determine continued medical necessity. Often used for higher levels of care.
Verification of Benefits (VOB): The process of confirming a patient's eligibility, coverage, copays, deductibles, and any preauthorization requirements before beginning treatment.
Zero-Pay Claim: A processed claim where the insurer determines no payment is due; usually due to unmet deductible, non-covered service, or incorrect billing.
